3. Appointments
All appointments are subject to:
- Clinical Appropriateness
- Resource Availability and Confirmation
- Professional Availability
- Operational Feasibility
ESH 800 reserves the right to reschedule appointments when necessary and doesnt guarantee immediate appointment confirmation as its solely based on availability.
4. Patient Obligations
Users agree to:
- Provide Accurate Information
- Disclose Relevant Medical History
- Follow Treatment Recommendations
- Treat Healthcare Personnel Respectfully
- Comply with Applicable Laws
5. Telehealth
Teleconsultation services do not replace emergency medical care, hospital admissions and intensive care services.
Patients experiencing urgent medical conditions should immediately contact emergency services or visit the nearest emergency facility.By participating in teleconsultation services, patients acknowledge that:
- Telehealth has inherent technological limitations.
- Certain conditions require physical examination.
- Technical Disruptions May Occur
- Healthcare professionals may recommend physical assessment or emergency care when clinically necessary.
Patients consent to the collection, use, storage, and transmission of information necessary to provide telehealth services.
6. Eligibility Of Service
To avail services from ESH 800, patients shall provide accurate information including but not limited to personal, medical, clinical eligibility for insurance, medication details, allergies, existing condition, etc.
Patients remain responsible for co-payments, deductibles, non-covered services and rejected claims and ESH 800 shall not be liable for any such matters for any legalities or payments
ESH 800 reserves the rights to decline service requests that are clinically inappropriate, unsafe, unlawful, has discrepancy in identification verification, out of scope services, etc
ESH800 Home Health LLC does not provide emergency medical services. In case of a medical emergency, please contact the appropriate emergency medical services immediately.
7. Professional Judgement
All clinical decisions, recommended treatment plans and their modifications remain the sole responsibility of licensed healthcare professionals.
Healthcare professionals may decline, modify, postpone, or discontinue services if deemed clinically necessary.
8. Intellectual Property
All website content, trademarks, software, graphics, text, logos, images, videos, documents, forms, materials, etc remain the exclusive property of ESH 800.
9. Home Visit Requirements
Home healthcare services are not substitutes for emergency medical treatment, hospital admission and intensive care services.
Patients receiving home healthcare services agree to :
- Provide and maintain safe access and environment
- Cooperate with Treatment and procedure and follow the instructions. May ask questions for any queries
- Provide accurate and complete information
10. Fees and Charges
Service fees with the applicable taxes may vary depending on the type of service availed, the clinical requirements, location, duration and frequency of service, doctor speciality, insurance coverage.
Payments may be excepted through card or cash, before or after availing the service based on the option selected, based on its availability service-wise
11. Website And Mobile Application Usage
Users agree not to:
- Misuse the Website / Application
- Attempt Unauthorized Access
- Introduce Malware or Harmful Code
- Violate Applicable Laws
- Interfere with Website / Application Operations
ESH 800 reserves the right to restrict access to users who violate these Terms.
12. Limitation of Liability
ESH 800 shall not be liable for delays or interruptions, indirect damages, consequential damages, loss of profits and/or business opportunities, etc caused by circumstances beyond its reasonable control.
